This position will have the opportunity for a hybrid schedule (a combination of in-office and remote workplace). **Please note - equipment provisioning and certain onboarding tasks will take place in person, in our downtown Denver office, on the first day of employment.
The Colorado Department of Early Childhood (CDEC) ensures the delivery of a comprehensive, community-informed, data-driven, high-quality, and equitable early childhood system that supports the care, education and well-being of all Colorado's young children, their families and early childhood professionals in all settings. CDEC provides access to collaborative, coordinated, quality early childhood programs and support to children, families, and early care professionals in an effort to best prepare Coloradans for future success. CDEC works with many partners, including parents, schools, child care providers, Early Childhood Councils, counties, Community Centered Boards, early intervention service providers, family resources centers, county human services, county public health, businesses, community organizations, and other stakeholders to provide high-quality early childhood and family support programs.
Description of Job
The Early Intervention (EI) program provides developmental support and services for children birth through two years of age who have special developmental needs. EI services can help improve a child’s ability to develop and learn as well as assist families by teaching new ways to promote a child’s development. The EI program provides services such as speech therapy, physical therapy, and other developmental services. EI is a voluntary program offered statewide and does not discriminate based on race, culture, religion, income level or disability.
What you’ll be doing:
This position is responsible for receiving referrals via the EI Colorado Referral Line, fax, and online referrals through the EI statewide Salesforce data system for families that have an infant or toddler with a diagnosed established condition or suspected developmental delay referred for evaluation to EI Colorado. As the initial service coordinator of record, this position is the first point of contact for families and is responsible for explaining the early intervention program and process for evaluation, family rights, eligibility, and service provision. This position is responsible for conducting the intake process and scheduling evaluation activities via the data system.
Major Job Duties include, but are not limited to:
+ Referral and Intake
+ This position is responsible for the timely response to referrals as part of a Referral and Intake Team that receives statewide referrals through the EI Colorado Referral Line, fax, and online via the EI Salesforce data system.
+ Acts as a first point of contact for families that have an infant or toddler with a diagnosed established condition or suspected developmental delay and serves as the initial service coordinator.
+ Explains early intervention and the evaluation process to families in a culturally responsive manner.
+ Conducts an intake questionnaire, provides information on the family’s rights, explains the evaluation process, utilizes developed protocols to determine the best disciplines to provide the multidisciplinary evaluation, enters child and family information into the data system, and schedules the multidisciplinary evaluation.
+ Sends referral status updates to the referral source.
+ Evaluation Scheduling
+ Works independently in a remote working environment to schedule evaluations based on family preference and provider availability. Accesses statewide Salesforce data system to enter information and determine provider discipline, location, and availability.
+ Manages the missed appointment and reschedule process.
+ Documents work activities.
+ Works with the Intake Manager and other Care Navigators to ensure a smooth and positive experience for referral sources and families seeking an evaluation.
+ Adheres to all protocols and scripts to ensure a consistent and high-quality experience for families.
+ Referral to Early Intervention (EI) Entities
+ Once a child is determined eligible or ineligible for early intervention services, completes a referral in the data system to the appropriate EI entity or, if ineligible, other resources to support the child and family.
+ Acts as a liaison to assigned EI entity to ensure a smooth transition (warm hand-off) to the receiving program and ongoing service coordinator.
+ Adheres to all required timelines to ensure that EI entities have ample time to begin timely services.

Minimum Qualifications
Six (6) years of full-time professional work experience in a field related to social work, political science, early childhood, human services, public administration, service coordinator, call center, or in an occupational field related to the job assigned to the position.
OR,
A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years ; sociology, psychology, social work, political science, early childhood, human services, public administration, or a closely related field is required.
