We are seeking an Early Intervention Service Coordinator to join our team at our program in Bensalem, PA.

 

Position Details:

Maintains a caseload of 50+ families with minimum monthly contact. Acts as a liaison with Bucks County MH/DP, Behavioral Health, Children & Youth, referring agencies, families, child advocacy representatives/organizations, and stakeholder service providers. Ensures program compliance with all internal and State/County policies, regulations, and requirements. Completes intakes in accordance with Merakey of Bucks County policies and procedures. Facilitates and participates in the assessment/evaluation, development, implementation, and review of IFSPs, as required by program regulations and family needs. Assesses child needs and family concerns. Secures pertinent information regarding child development. Coordinates implementation of intervention services. Educates families on outcomes and the family-training, routines-based model of intervention. Assists families in identifying available service providers and facilitates communication between the family and service provider.
