**POSITION SUMMARY:**

**Early Intervention Services is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. The EIS Lead is a professional who collaborates with physicians, health care team members and patients to coordinate care for specific patient populations across the continuum. Provides various social work services to meet needs of patients including leading groups and mental health crisis interventions in addition to routine medical case management activities: biopsychosocial assessment, care plan, insurance navigation and linkage to other resources necessary for patient engagement in care. Provides mentorship, guidance, and support to other EIS case managers. Social work practices and services are consistent with applicable accreditation and licensing standards and reflect the National Association of Social Worker Code of Ethics.**
**What you will do:**
**1. Independently or in collaboration with other MSW, leads monthly support groups including:**
**• Planning & Implementation**
**• Providing direction and guiding discussion**
**• Creating and following relevant policy/procedure**
**• Providing guidance and support group facilitators.**
**2. As Lead, supports and guides EIS**
**colleagues. Collaborates with manager,**
**colleagues and Quality Mgt. on policy/process**
**improvement. Includes but is not limited to**
**support and guidance around grant requirements,**
**funded activities, performance measures and**
**quality management/improvement activities for**
**the EIS program.**
**3. As Lead, provide routine supervision support for**
**EIS colleagues and other social workers on the team.**
**This includes professional supervision for BSW/MSW**
**licensing requirements.**
**4. Conducts biopsychosocial assessments and**
**reassessments with patients at defined intervals.**
**and completes comprehensive, individualized**
**Care Plans with clients on their caseload.**
**5. Provides case management services in alignment with**
**grant requirements and client support needs with the goal**
**of supporting client engagement in HIV care.**
**6. Works collaboratively in a multidisciplinary team environment fostering.**
**What you will need:**
1. Masters of Social Work. earned within 6 months of hire, prior employment experience in a
related area preferred.
2. Upon hire candidate must have a Bachelor’s degree in Health or Human Services from
accredited College or University. Prior employment experience in a related area
preferred.
3. Demonstrate ability and skill in the management of patient care planning and continuous
quality improvement in services delivered.
4. Demonstrated ability to engage in consistent communication with members of the patient
care team; participation in case conference or other collaborative meetings where
decisions about patient care are taking place.
5. Well-developed communication skills, both written and oral. Ability to communicate
effectively with individuals and groups representing diverse perspectives.
6. Ability to work flexible work schedule.
7. Strict adherence to patient confidentiality expectations.
8. Knowledge of applicable accrediting and regulatory standards.
9. Articulates and demonstrates commitment to the philosophy, values, mission and vision of MHP
