**Overview**
**Fairview Health Services is hiring an Employee Occupational Health & Safety Nurse Case Manager.**
The MHealth Fairview Employee Occupational Health & Safety (EOHS) Nurse Case Manager is responsible for the assessment, planning, delivery, and evaluation of professional nursing care provided in response to potential work-related illness, injuries, and incidents. The EOHS Nurse Case Manager provides coordination of care, case management, and facilitates a safe and timely return to work for employees. The EOHS Nurse Case Manager has primary responsibility for care and services during clinic hours. This position is mainly remote with the potential of coming onsite.
**Schedule:** This position will be working 1.0 FTE (80 hours every two weeks). This position is a mobile position will require both remote and on site requirements. Monday-Friday 8-4:30 PM or 7:30-4:00PM hours
**Responsibilities Job Description**
+ Responsible for the triaging and managing of employee events.
+ Responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.
+ Active injury case management, care coordination and communication including report intake, return to work activities, associated work restrictions, and follow-up.
+ Provides subject matter expertise and guidance regarding benefits, entitlements, support programs, disability management best practices and responsibilities of stakeholders.
+ Communicates and coordinates case management activity with injured worker and appropriate individuals including Leaders, Human Resources, third party administrator, QRC, physician, RISK, legal etc.
+ Educates staff on work injuries, ADA, reasonable accommodation, and transitional work program.
+ Provides employee occupational health services through evaluation and facilitation of appropriate medical care for injured/ill employees that improves quality and cost effectiveness of both healthcare and employee return to work.
+ Collaborates with Employee Health & Safety team to implement and coordinate injury prevention.
+ Facilitates return-to-work activities for occupational and non-occupational cases.
+ Maintains comprehensive documentation of work, ensuring accuracy and reliability of data using software that is available.
+ Ability to provide direction and consultation regarding disability, return to work, WC, interaction discussion process and accommodation decisions to leaders and other stakeholders.
+ Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards.
+ Actively participates in creating and implementing improvements.
+ Fosters and maintains supportive relationships with internal and external stakeholders.
+ Responsible for maintaining OSHA logs and reporting.
+ Responsible for reporting to Bureau of Labor Statistics (BLS)
+ Performs other responsibilities as needed/assigned.
**Qualifications**
+ Minimum two-year (AAS or AD) college nursing degree
+ Minimum four years work experience in occupational health nursing or in an area of nursing requiring independent nursing skills (i.e.: ED nursing, Medical Surgical, Mental Health, etc.)
+ Minimum computer skills must be sufficient to efficiently work with email, Excel, electronic medical records, various other on-line systems, etc.
+ Work off hours under special conditions.
+ Current MN RN licensure with the ability to be licensed in other states
**Preferred Qualifications**
+ BSN or BAN degree - four-year college nursing degree
+ Nurse Case Management experience
+ RN with more than four years of experience or a broad understanding of hospital EOHS processes, case management, ADA, and workers compensation.
+ Lean Six Sigma Certification
+ Current certification as COHN-S, COHN, COHN-CM
