Overview
Oversee the daily operations of the AGS CoCare: HELP Program includes screening and enrolling patients into the program. Orients patients and families to the program and assign appropriate intervention protocols. Assumes direct responsibility for the Daily Visitor protocol, Therapeutic Activities protocol, Early Mobilization protocol and Feeding Assistance protocol. Assist with recruitment, screening, and placement of program volunteers. Assumes direct responsibility for training and retention of program volunteers. Functions as clinical expertise in the delivery and evaluation of geriatric nursing care.
Responsibilities Daily screening and enrollment of appropriate patients into program. Screening procedure includes reviewing medical record and interviewing and orienting patient/family and staff nurse. Develops and initiates individualized care plans of AGS CoCare: HELP intervention protocols. Provides appropriate equipment. Updates care plan as needed based on patients' needs and outcomes.Notify AGS CoCare: HELP staff, nursing staff and other appropriate healthcare staff of enrollment status. Serve as a clinical resource and as a role model regarding geriatric nursing care. Implement intervention protocols by assigning patient-appropriate activities to the volunteers. Assures adequate volunteer and staff coverage for all interventions and activities, including evenings, weekends, and holidays.Collaborates with Guest/Volunteer Services to establish a recruitment plan for AGS CoCare: HELP volunteers. Coordinate and direct quarterly Volunteer training program.Perform quarterly Quality Assurance measurements on volunteers’ interventions to assure interventions being correctly and consistently implemented. Provides routine supervision and support of volunteers with ongoing feedback.Records and monitors adherence of all volunteer interventions, providing progress reports as needed.Monitors and records adherence to all protocols and program outcome variables.Distributes, collects, and collate results of Patient Satisfaction Surveys.Tracks all equipment in patient use and cleans and maintains equipment in working condition. Qualifications Bachelor of Science in Nursing (BSN) required.Typically requires 3 - 5 years of experience in clinical nursing including experience in providing care for geriatric patients.
