SUMMARY: The Participant Escort will meet and assist ElderONE participants at their scheduled appointments.

STATUS: Part Time               

LOCATION: Elder One       

DEPARTMENT: Clinical Operations

SCHEDULE: Days      

ATTRIBUTES:

Reliable transportation to meet participants at scheduled appointments

Experience with elderly population preferred

Experience with direct patient care preferred

HHA, CNA, PCA preferred

RESPONSIBILITIES:

Complete activities as listed within the participant care plan as required during each encounter

Meets participants at scheduled appointments

Assists participants with checking in and checking out for appointments

Provides assistance with needs while attending the appointment including (but not limited to): supervision, transfers, dressing and undressing, toileting, assist with positioning

Communicates with Appointment Coordination team and Transportation to review any cancellations, no shows, change in appointment times

PAY SUMMARY:
The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
