Salary Range: $27.52-$41.27/hour
BASIC FUNCTION:
The EMG Social Worker provides initial assessment, psychosocial history and therapeutic interventions to select patients in the primary care office. The Social Worker will also provide consultative services to the primary care staff including establishing a psychiatric diagnosis, making recommendations for treatment, and linking patients to psychiatric, substance abuse, and other social services.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Education:
A Master’s degree in Social Work. Licensed Masters Social Work (LMSW) required. LCSW preferred.Experience Required:
One year of practice in a primary care and behavioral health setting.

PHYSICAL REQUIREMENTS:
Should be able to push/pull, lift/move 35 lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to be stationary at a workstation for a prolonged period time in addition to being able to squat or be mobile for a reasonable length of time and distance. Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.PRIMARY RESPONSIBILITIES OF THE POSITION:
Provides clinical services including brief counseling and crisis intervention and referral for follow-up services for select patients of the primary care practice. Evaluates patients for risk and dangerousness to self or others. Assess, plan, implement and evaluate individual healthcare needs. Accurately assesses patient needs and offers a variety of appropriate treatment interventions. Assists patients in obtaining ancillary services when needed. Maintains contact with other agencies and families as needed. Manages liaisons with community service providers as needed. Cross serves palliative and behavioral health patients. Completes required documentation. Participates in program improvement initiatives as appropriate. Adheres to hospital policies and procedures related to mandatory education, annual health assessment. Provides consultative services to the Doctors and residents of the practice.

Salary Range: $27.52-$41.27/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
