**Role Purpose**
Provides support services and tailored solutions to address the psychosocial needs that team members may encounter at the designated HSO(s) (Healthcare Service Organization). Committed to understanding and meeting the unique needs of the community within which our team members operate, fostering a safe environment that promotes overall well-being and productivity.
**Responsibilities**
+ Partners with management and employees in assigned work areas to provide community support services to team members, using interventions that result in positive outcomes, based on individual strengths, and needs; case load sizes will vary.
+ Responsible for capturing and tracking key data. Monitors and analyzes data to report out and provide BJC leadership with proactive consultation and recommendations.
+ Outreach with team members in assigned work areas through leadership, huddles, rounding, and staff meetings. Provide education about available resources in various venues, including presenting to larger groups of team members on topics that are relevant to identified needs, such as navigating resources, managing stressors, etc.
+ Builds relationships with agencies and resources in the community and build linkages to those resources for team members. Ability to network with community leaders.

+ Bachelor's Degree - Social Work/Human Srvc/Related
**Experience**

+ Master's Degree
