Position Overview:
Provides Mental Health Screener functions and performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments.
Consults and collaborates with medical center staff to coordinate interdepartmental patient services. Develops comprehensive and realistic treatment plans based on patient's needs.Provides short term follow-up to ensure linkage and stabilization. Provides advocacy and support to patients to secure treatment and services.Engages families, treatment providers, and other parties active in the well-being of the patient whenever possible. Provides technical support, assistance, and education to patients, families, and other patient service providers.Assumes the duties of charge person when required.Assumes the duties of mobile outreach on-call when required.Documents all telephone triage information; clinical assistment data; changes in mental status; and all communications related to patient referrals, transfers, and admissions in the patient record.Offers are contingent upon successful completion of our onboarding process and pre-employment physical.
