ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With Senior Life, patients have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they’ll get the coordinated care they need. PACE provides and coordinates care at our Wellness Centers, in patient’s homes and in other settings.

Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data. (2-3/days for Full Time position)Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan.Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care. Communicates with the Care Team and participates in interdisciplinary team meetings.Assessments will be conducted in all service areas. Reliable transportation required.Responsible for environmental assessments.Completes semi-annual assessments as requestedMaintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
