JOB SUMMARY:    The ER Care Coordinator RN uses InterQual criteria to evaluate the appropriateness of admissions, concurrent stay and readiness for discharge.  She/he assures the correct level of care: critical, telemetry, acute, observation, for all patients.  The Care Coordinator provides an efficient, coordinated approach to clinical management. The essential functions of the Care Coordinator require constant collaboration with the Social Workers, Nursing staff, Financial Counselors, Medical Staff to coordinate the care of patients in a proactive manner. 

When there are no admissions or transfers etc. the Care Coordinator will function as an ERRN to assist the ER staff as directed by the charge nurse, He/She will not be given an assignment.

The position requires the full understanding and active participation in fulfilling the mission of GMC - Garfield Medical Center.  It is expected that the employee demonstrates behavior consistent with the core values.  The employee shall support GMC - Garfield Medical Center strategic plan and goals and direction of the performance improvement plan.

 

 

EDUCATION, EXPERIENCE, TRAINING

 

RN License in the State of California required. Certified Case Manager preferred. Meets education and competency requirements for position.BCLS Card AHA required.ACLS AHA requiredMAB-AB508PALS

NIHS
