Position Summary Full-Time Day Shift $20.99 - $32.54

Coordinates all aspects of the Outpatient Cardiac and Pulmonary Rehabilitation Program (Phases II and III) including exercise prescription, exercise leadership and education.

Interviews and evaluates referred patients according to Cardiopulmonary Rehab policy and procedures. Develop appropriate goals and plan along with each patient. Re-evaluates goals and plan based on changes in patient’s status. Observes, records and reports to referring physician treatment plan and response to interventions. Provides exercise therapy using appropriate modalities in order to progress toward established goals. Monitors responses to interventions using physical assessment and signs/symptoms.  Provides education and support for risk factor modification.  Reinforces as necessary. Documents as required. Leads supervised exercise sessions and provides training on daily rehab routine, proper use of equipment and proper exercise techniques. Provides guidance for home exercise program and selection of home exercise equipment. Assists with emergency care according to department policy. Participates in the development and implementation of special projects, new programs and community-sponsored events. Conducts and modifies as necessary educational programs for patients and staff. Participates in professional continuing education. Supervises and educates student interns. Maintains effective communication with patient, family members, physicians, hospital staff and the public. Monitors cleanliness and maintenance of equipment and reports any deficiencies. Participates in community health screenings and educational events. Participates in QI activities of the department as needed. Helps coordinate and promote the transition of patients to the Phase III program. Adheres to behavioral standards. Performs other related work as required or requested.

Required Experience

Education:

Bachelor’s Degree in related field required Master’s degree in Exercise Physiology preferred

Licensure/Certification/Registry:

Certified through ACSM as an Exercise Specialist (preferred but not required). Current CPR certification. Current ACLS certification. Member of professional organization desirable.

Experience:

Experience in exercise program planning, supervision, and counseling with cardiovascular and pulmonary patients.

Other Knowledge/Skills/Abilities:
