Family Advocacy and Support Services Worker (Per Diem)
The Opportunity
The Caregiver/Family Advocacy Support Services Worker provides home and community-based individual and/or group Advocacy and Support Services to families/caregivers and children to offer educational, advocacy and support resources to develop family/caregivers' ability to independently access community services and improve the child/youth's ability to gain from the community experience.
Responsibilities
+ Provide Caregiver/Family Advocacy and Support Services as established with the family/child on the child's treatment plan.
+ Educate and train the caregiver/family unit on available resources to assist them in better advocating for the needs of the child/youth and accessing needed services.
+ Train child/family in self-advocacy, including during community transitions.
+ Train child/family on methods and behaviors to enable success in the community.
+ Adhere to principles of Individualized Care Model and base activities on the needs, abilities and strengths of each child and family.
+ Develop and implement treatment and safety plans in conjunction with the child/family ensuring plans are updated according to changing needs of child/family and state guidelines.
+ Document visits and contacts with clients through progress notes that are submitted within 24- 48 hours of visits.
+ Maintain communication with care managers regarding client progress.
+ Other duties as assigned by supervisor.
Qualifications
+ Bachelors Degree with 2 years of related experience or Master's Degree in Human Services or Education with 1 year of experience.
+ Excellent verbal and written communication skills.
+ Valid New York Sate Driver's License
Additional Information
Schedule: Up to 19 hours per week. Weekday and Weekends, Afternoon and Evenings.
Salary: $20/hr
