disabilities.
SRVS is one of the largest premier disability provider agencies in Tennessee with
programs available in Davidson, Montgomery Shelby, Hardeman, Haywood, and other
surrounding counties in Middle and West Tennessee.
SRVS is currently seeking:
A caring and compassionate individual or family to welcome an adult with
intellectual/developmental disabilities into their home and care. If you want to make a
positive difference in someone’s life and an impact in your community, become a Family
Model Provider in Middle Tennessee.
SRVS Family Model Providers (FMPs) are contracted and reimbursed at a per diem rate.
In addition, FMPs will receive monthly room and board.
REQUIREMENTS:
High School diploma or GED.
Must have experience working in the field of disabilities or related area.
Must have a valid driver’s license.
Must be able to pass a drug screening and criminal background/DMV checks.
Responsibilities:
Demonstrate dignity and respect in all interactions with the person(s) supported.
Ensure that the person(s) supported is/are supported and encouraged to make their own
decisions, are provided choices, and are provided with a meaningful day.
Understand that “on duty” time for a Family Model Provider is defined as 24/7 for 365
days a year.
Ensure that the person(s) supported are provided a safe environment both inside and
outside of the home.
Ensure that the person(s) supported lives a healthy lifestyle.
Comply with rules of confidentiality as defined by state/federal law, DDA/MCO and
SRVS.
The Family Model Provider agrees to participate in the training as required by
DDA/MCO and SRVS for compliance purposes or specialized training for a person’s
specific needs.
The Family Model Provider understands and agrees that this is a CONTRACT position,
and no company benefits are provided, i.e., insurance, earned vacation, or sick leave. In
addition, transportation of the person(s) supported is also a part of the contractor’s
payment and is not billed separately.
The Family Model Provider agrees that as an independent contractor of SRVS they must
comply with SRVS Policy and Procedure Manual.
