Job Title: Family Case Manager I/II/III
Department: Clare Swan Early Learning Center
Reports to: Family and Community Services Manager
Supervises: None
FLSA Status: Non-Exempt

General Functions:
The Family Case Manager’s primary responsibility is to build strong, supportive partnerships with families to help them set and achieve family-centered goals. This role ensures families receive coordinated support across key areas, including education, child development, health, nutrition, disabilities, mental health, and access to community resources. Working collaboratively with internal departments and adhering to Head Start Performance Standards, the Family Case Manager also plays an active role in continuous quality improvement (CQI) efforts to promote positive outcomes for both children and families.
Duties and Responsibilities, including but not limited to:
Family Case Manager I
Develop strengths-based relationships with assigned families to foster active family engagement in their children’s education, development, and overall well-being.Support seamless transitions for children entering preschool by coordinating with partner preschools and ensuring continuity of care and education.Partner with the Education team to meet home visit and parent-teacher conference objectives, promoting active family involvement in child development activities.Support the Education team with two announced home visits per child per year.Conduct unannounced home visits, as necessary, to address attendance concerns and support consistent participation.Monitor and support attendance to maintain a minimum rate of 85% for assigned families. Collaborate with families to identify and address attendance barriers and implement Attendance Agreements when necessary.Maintain comprehensive knowledge of community resources and provide referrals for services including TANF, housing, childcare, CITC programs, and other relevant support systems to meet family needs.Deliver crisis intervention services as needed, in alignment with the program’s philosophy and under the direction of the Family and Community Services Manager.Assist in planning and implementing family engagement initiatives, including monthly family nights and parent education workshops, to strengthen family involvement and community connection.Accurately document all family interactions, referrals, and services by the end of each business week to ensure timely and thorough recordkeeping.Support program recruitment by actively participating in community outreach efforts and events to raise awareness and promote enrollment.Collaborate with the ERSEA Coordinator to facilitate New Student Orientations and assist in resolving attendance or enrollment challenges as they arise.Collaborate with the CSELC team to analyze data from community and self-assessments, identifying trends and opportunities to inform and enhance program improvement efforts.Family Case Manager II
Perform all Family Case Manager I responsibilities a high level of proficiency. Create and develop specialized family support materials, such as resource guides, child abuse prevention tools, and parenting education content, for distribution to enhance family engagement and support.Gain proficiency in prenatal programs, family engagement initiatives, and Yugtun family events to effectively support and engage families.Provide comprehensive crisis intervention services as needed, working closely with the Family and Community Services Manager and other specialists to ensure effective support and resolution.Take a central role in family engagement efforts, ensuring active family participation in program initiatives that promote and support child development goals.Support families of children with disabilities by collaborating with the Disabilities team to ensure adherence to IFSPs and access to other relevant services.Family Case Manager III
Lead the duties and responsibilities of Family Case Manager I & II, with a high level of proficiency.Collaborate with the management team on community and self-assessment results, identifying trends and implementing solutions to improve family services.Mentor Family Case Managers I & II, providing support and guidance to ensure consistent delivery of family services.Organize and facilitate training for Family Case Managers I & II, ensuring they have the resources to support families effectively.Lead the planning and facilitation of parent education and community engagement events, based on parent interest surveys, grant objectives, and Head Start performance standards.Serve as a liaison between families and other program areas (e.g., Health, Disabilities, Mental Health, ERSEA), ensuring seamless service delivery for children and families.Provide direct support to the Senior Director in managing Parent Policy Council activities and initiatives.Respond to crisis prevention and intervention needs for all center families, ensuring alignment with program philosophy and goals.Minimum Core Competencies: CITC Values, Respectful Leadership, Professionalism, Emotional Intelligence, Problem Solving/Critical Thinking, Communication Skills.
Job Specifications for All Levels:
Excellent oral and written communication skills.Exceptional public communication and telephone etiquette, with a professional and approachable presence.Proficient with computers: word processing, email communication, internet resourcing, spreadsheets, and childcare management software. Excellent organizational skills, attention to detail, and time management skills.Proven ability to coordinate and manage multiple activities simultaneously.Strong understanding of child development and parent education principles.Knowledgeable in applying relevant philosophies, including Strengthening Families, and understanding the impact of trauma, particularly Adverse Childhood Experiences (ACEs).Flexibility in the work schedule is required to meet with participants in various settings (home, community, office).Provide support to meet classroom ratio as needed. Maintain confidentiality according to regulations, policies, and procedures. Perform other duties as needed and assigned.
Minimum Qualifications:
Case Manager I
Associate degree in social work, Human Services, or Early Childhood Education. Relevant experience may be substituted for the education requirement on a year for year basis.One (1) year of experience with Case Management.Case Manager II
Associate degree in social work, Human Services, or Early Childhood Education. Relevant experience may be substituted for the education requirement on a year for year basis.Two (2) years of experience with Case Management.Case Manager III
Bachelor's degree in social work, Human Services, or Early Childhood Education. Relevant experience may be substituted for the education requirement on a year for year basis.Two (2) years of experience with Case Management.Minimum Qualifications for all Job Levels:
Infant/Toddler CPR/First Aid certified or become within 60 days (about 2 months) of hire.Demonstrated knowledge and understanding of the social, health, educational, training, and cultural needs of the Alaska Native and American Indian community.Continued employment is contingent upon satisfactory completion of state and federal background checks.Must provide copy of current Health Report.Valid Alaska driver’s license and be insurable under CITC’s automotive insurance, which requires a driver to be at least 21 years of age and have had a driver’s license for at least three years.Preferred Qualifications:
Case Management experience in Alaska, specifically working with Alaska Native and American Indian cultures. Pre/Post Natal family support experience such as a doula.Two (2) years of Head Start Experience, including knowledge of ChildPlus software. Able to get up and down off the floor with ease, including kneeling, bending, squatting. The ability to wear the appropriate PPE. Frequently exposed to bodily fluids. Possible exposure to noise, and contaminants such as cleaning solvents. Routinely exposed to outdoor conditions including inclement weather.
