About this role:
As a Family Medicine Oncology Survivorship Physician, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. As a Family Medicine Oncology Survivorship Physician, you will be an integral part of a multidisciplinary team dedicated to delivering high-quality, compassionate care to oncology patients transitioning into and navigating through the survivorship phase of their healthcare journey. In this role, you will:
Serve as a coordinator of care, ensuring seamless communication and collaboration among specialists, primary care providers, and support services.Monitor for recurrence and manage long-term physical and psychological effects of cancer and its treatment.Address and manage co-morbid conditions that may impact survivorship.Promote healthy behaviors and implement preventive care strategies to support long-term wellness.Provide patient-centered care that emphasizes education, empowerment, and quality of life.This role requires a compassionate, proactive approach to care, with a strong emphasis on continuity, prevention, and holistic support.
This position includes a $18,750.00 sign-on bonus, $15,000 in relocation assistance, stipend and $75,000 in student loan repayment when applicable

What you’ll bring:
