Family Team Conference Facilitator/PYA Specialist

Position Overview:

The PYA/FTC  Specialist ensures that direct care staff and program participants learn, implement, and engage in positive independent livings skills, and that these skills are taught in an effective therapeutic milieu.   Utilizing best practices in their work with youth, the PYA /FTC Specialist will ensure that all youth in the AOBH’s receive weekly and monthly workshops that will help them to prepare to exit the foster care system and/or transition into adulthood in a positive and healthy manner.  The primary role of this position is to establish a healthy relationship with program staff and youth and provide guidance, modeling, and opportunities for youths, to learn social and independent living skills The PYA /FTC Specialist must always act as a mature and positive role model to create and/or enhance the overall therapeutic environment.  The PYA Specialist must work closely with other team members and with community resources as needed.  This is a dual role, The PYA/FTC Specialist will also be responsible for conducting and documenting in connection all permanency FTC’s.

Position Qualifications:
