The RN Field Supervisor is responsible for the implementation of the nursing process for the patient population which she/he services, through case management, development of a care plan, clinical intervention and follow-up.
Responsibilities
Makes initial and ongoing Supervisory Field Visits to patients’ home to provide supervision, assessment, training, support, in-home coordination and/or crisis management. Reports all interventions made to the Clinical Manager. Conveys any significant changes in the patient’s condition, emergency interventions made or changes in the care plan to the Clinical Manager immediately and documents findings electronically.
Gathers the patient’s medical history information, performs head to toe assessment, and collects medication data for the purpose of complying and conferring with the Primary Care Physician and the Clinical Team/Clinical Manager.
Supervises performances of procedures done in the home for the first time by the Home Health Aide/Per-diem provider. Identifies continued training needs of the Home Health Aide and documents this information electronically.
Delivers to the home a complete in-home information packet on initial visit and reviews the Paraprofessional Plan of Care with the HHA. Instructs the HHA on Special Circumstances with return demonstration by the aide when needed.
Performs in the role of Clinical Manager on call or Field RN on call as is required by departmental needs and determined by the Clinical Manager, and is compensated for this service on a weekly basis.
Performs in the role of direct care provider during regular hours or while serving as Field RN On-call, at the direction of the Clinical Manager, or the Clinical Manager On-call. Receives standard per diem compensation for these services when provided during non-regular hours.
Participates in the Quality Assurance Program and Education Program as requested by the Clinical Manager.
Performs related duties as assigned.
Qualifications
EDUCATION: Associates in Applied Science Degree minimum, Bachelor’s Degree preferred.
EXPERIENCE: Preferred (2) years clinical background as an RN, and/or prior home care experience in a Licensed Home Care Service Agency or CHHA setting with the following experience preferred.
OTHER: Licensed as an RN to practice in the State of New York.
