We are passionate about what we do because our care matters. **Overview** We’re looking for a **Licensed Practical Nurse (LPN) Continuous Care** to join our team in **Ontario, OH.** This position will report directly to the Executive Director/Administrator or Director of Clinical Services, but work under the direction of the Registered Nurse to provide continuous care visits to patients and families in a facility/unit or home setting.
