Geriatric Nurse Practitioner (APRN)
*Description* Affiliated with Danbury Hospital, Nuvance Health Medical Group is seeking a full-time *Geriatric **Nurse Practitioner (APRN) for a dual role *with estimated 50% patient facing care hours and estimated 50% performing inbox management. ** This newly created role includes* *working to provide primary care access for Meadow Ridge in the Skilled Nursing Facility, the Assisted Living Facility and in the Primary care office located on site in Redding, CT. In this role, you will also work to support inbox management - providing coverage for prescription refills, reviewing patient labs and imaging results and portal messages for clinicians on pto, leave or who have retired. Meadow Ridge is a luxury continuing care retirement community in Redding, CT offering Independent Living, Assisted Living, Memory Care, and Skilled Nursing. The position will be onsite Mon-Fri. Hours can be tailored to best support a work-life balance while still meeting the needs of the residents. A three-month mentorship program is part of your initial training experience. *Job Responsibilities:* The APRN provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses. The APRN also works to support inbox management - providing coverage for prescription refills, reviewing patient labs and imaging results and portal messages for clinicians on pto, leave or who have retired. *Job Requirements:* * This position requires a minimum formal education or master’s degree, NP, State of CT RN and APRN license, CT Controlled Substances and Federal DEA Registration and Board-Certified Nurse Practitioner. AGNP preferred but willing to consider FNP candidates with geriatric experience. * *At least three years’ experience as an APRN in Primary Care/Geriatrics required.* ** This is an employed position with Nuance Health Medical Group.
