POSITION SUMMARY:
The Diabetes Fitness Trainer, under the direction of the Diabetes Program Manager, is responsible for the assessment of physical fitness needs/abilities of individuals or groups with Diabetes Mellitus, Prediabetes, or Chronic Illnesses. This position will develop and demonstrate exercise & physical activity routines while assisting by monitoring and modifying activities as needed to minimize injury and promote fitness. This position is responsible for working with the Diabetes Program team to plan, organize, implement and evaluate the physical fitness portion of those with Diabetes Mellitus.
ESSENTIAL FUNCTIONS: (includes, but is not limited to, the following)
Consults with individual patients as referred by their Primary Medical Provider to formulate a physical fitness plan. Analyze patients' medical history to determine the best possible exercise and fitness regimen. Perform fitness tests with medical equipment and analyze the subsequent patient data. Measure body fat and other key patient health indicators. Develop exercise programs to improve patients with Diabetes Mellitus and other chronic health issues. Supervise clinical tests to ensure patient safety. Charts and files all patient reports within Electronic Medical Records. Provides health and fitness education in-service training for staff as needed. Prepares presentations, leads group events, and organizes and chairs meetings as directed. Participates in performance improvement activities.ADDITIONAL RESPONSIBILITIES: (includes, but is not limited to, the following)
Attends all required Health Care in-service training activities. Serves as a resource to assist individuals, other professionals, or the community as needed. All other job-related duties as assigned.CONTACTS:

PHYSICAL REQUIREMENTS:
Position medium with the lifting of 50 pounds maximum and frequent lifting/carrying up to 25 pounds. Physical factors include constant standing, use of hearing, near vision, depth perception, color vision; frequent walking, stooping, kneeling, reaching, midrange/far vision, field of vision, driving, typing, bending and occasional sitting, carrying/lifting, pushing, climbing, crawling, manual handling and use of smell. Working conditions include frequent exposure to noise and occasional exposure to weather, extreme heat/cold, wet/humidity, vibration, and air quality. Potential hazards include constant patient/client contact; frequent exposure to moving mechanical parts and computer use and occasional exposure to chemicals, infectious exposure, and medical equipment. Protective equipment/clothing is used as standard precautions with possible exposure to bodily fluids due to the risk of injury from patients’ physical activities/fall risks.
REQUIREMENTS:
Education: Bachelor’s degree in physical education, Exercise Science, Health Science, Nutrition, or Nursing required; will accept related field.
Experience: One year of experience working in Physical Fitness training or instruction preferred.
Certification/License: American Society of Exercise Physiologists (ASEP) or the American College of Sports Medicine (ACSM) Certification is required within 2 years of hire. CPR/First Aide Certification required. Will be required to undergo and successfully pass a background check. Must maintain a good moral character standing. Will be required to complete and pass pre-employment drug testing.
Knowledge, Skills, and Abilities: Knowledge of HIPPA standards and regulations is required. Knowledge of standard safe and healthy work procedures is required. Knowledge of national, state, and local fitness training standards and resources required. Must be able to demonstrate how to carry out various exercises and routines for patients that have health concerns such as Diabetes, Cardiovascular disease, or obesity. Must be able to watch and support patients by teaching individualized fitness plans. Must be able to offer alternative exercises during fitness plans and/or classes for different levels of fitness and skill. Must be able to communicate effectively, verbally and in writing in a diverse range of audiences and settings. Must have excellent communication skills and be sensitive to cultural and economic diversity. Must have excellent organization skills, organizing work, prioritizing tasks, and managing time. Must be able to work with minimal supervision. Must be able to establish and maintain effective communication with co-workers, supervisors, and the general public. Must be flexible and available to work various shifts. Must maintain confidentiality. Native American preferred.
