UBHC’s 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education. Posting Summary Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for the Brief Treatment – NJ Promise (Clinical Risk for Psychosis – CHRP) Department within Rutgers University Behavioral Health Care.

The Habilitation Counselor provides community-based case management services for young people at clinically high risk for psychosis to include assessment, treatment/recovery planning, linkage, and coordination of care.

Among the key duties of this position are the following: Provides case management and community support services to assist individuals (ages 10-25) and/or families to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services. Provides crisis prevention, intervention, and resolution services to individuals and families served. Provides training to individuals served and families/significant others on the recognition of relapse triggers and strategies to deal with avoiding a relapse. Demonstrates the ability to engage and establish a working relationship with and provide services to individuals with physical health, mental health, and substance use disorders. Provides specialized addiction treatment/co-occurring services. Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for individuals served and families. Maintains appropriate records as mandated by University Behavioral Health Care policy and applicable accrediting organization standards.
