Health and Well-being Specialist

The UPMC Jameson Child Advocacy Center is hiring a Health and Well-Being Specialist to join the CAC Team! This full-time role will work primarily daylight hours five days per week with occasional evenings and weekends required. This position requires travel within Lawrence County.  

This role will be contacting local pediatrician offices to obtain medical records for clients enrolled in the program as well as provide staff with health and wellness information.

Purpose:
Responsible for curriculum planning, family goals development, and implementation of the Parents as Teachers program with a caseload of infants/toddlers and their families. Also responsible for monitoring the accuracy and completion of family, child and health services requirements and assisting Family Development Specialists in accurate responsive case management and oversight of PAT early learning services.

Responsibilities:

Provide research-based home visitation with the schedule dictated by the program funder. Visits may require evening or weekend hours. Promotes parent involvement in working with their children. Provides direct service to identified families and their infants/toddlers including children with disabilities through home visits and organized small group meetings/socializations.  Maintains caseload of 8-12 families.  Responsible for obtaining health and dental records for enrolled children. Responsible for reviewing child health records, including the PAT health overview form to determine the health status for children enrolled and entering information into Penelope data base.  Coordinates appropriate follow-up for determined needs as identified on health, nutrition, and developmental histories as well as the child’s health and dental exams. Assist the parents and Family Development Specialist in obtaining further diagnostic testing, examination, and treatment for children with suspected or diagnosed health, nutrition, or developmental concerns.  Assist families to identify and access community treatment, support, and other resources. Develops and maintains partnership agreements with community agencies for support and services that benefit the children and families.  Assist in training staff in PAT required hearing and vision screening, tracking the completion and results. Provides monthly updates/progress reports including but not limited to services reports, program compliance, program quality, progress towards program goals and objectives, and others as deemed necessary. Works in conjunction with other program content areas to ensure a holistic approach and assessment of the quality of services provided. Monitors, analyzes, and tracks related data. Performs other duties as assigned.
