Health Associate| Behavioral Health Resource Center | Full-Time
Overview The BHRC Associate provides direct community health services through St. John's Care Connect to vulnerable populations through identification, engagement, and provision of resources to address unmet social and health needs. Responsibilities Provides direct community health services including individualized patient education, counseling, and referrals. Conducts client assessments including NEEDS Assessment and the Universal Intake Application to identify individual needs and develop service plans as indicated. Utilizes customer services skills, develops rapport, and builds trusting relationships with clients to further facilitate provision of services. Determines patient eligibility for hospital-based services including the Medication Assistance Program and Medical Voucher Program. Utilizes ServicePoint tracking system to follow all stages of client interaction. Collaborates in daily huddle to review clients with unmet needs and develop case plans to coordinate resources. Serves as liaison to community organizations aimed at improving the health and well-being of the region. Assures optimal patient flow by monitoring St. Johns Care Connect processes and report findings to supervising entities. Performs all other duties as assigned by management within job scope. Qualifications High School Diploma/Equivalent is required. 2 years of experience working with vulnerable populations is required. BLS, provided by the American Heart Association (AHA), required within 90 days of hire. Level 2 DCF Background Screening required upon hire. NAPPI (WELLE) Certification required within 90 days of hire.
