This position is a fully remote position that requires being a Registered Dietitian, CDCES in CST, MST, or PST time zones. The health coach will need to commit to working at least 2 late days per week (Monday-Thursday until 7 pm CST or their time zone) and at least one late Friday a month until 6 pm CST or their time zone.
Position Purpose: Assists members by completing health assessments and implementing effective solutions and care plans to improve their health and wellness. Works with members who are at risk or have been diagnosed with chronic condition(s) to provide support, encouragement, and education.
Utilize applicable clinical training and knowledge to provide appropriate health support and solutions to members with chronic health conditionsEvaluates the needs of the member by performing telephonic assessments to determine clinical risk and readiness to changeProvides telephonic support and coaching for members on ongoing wellness care plans and wellness programs to improve member’s health and lifestyle through outreach and educationAssists members in developing attainable wellness goals through plans, support, encouragement, and resourcesMonitors wellness plans and/or member status and outcomes for changes in care and provides recommendations to care plan based on member needs and issues identified to reduce health risksEngages and influences members in decisions relating to achieving and maintaining an optimal healthy lifestyleInteracts with healthcare providers as appropriate to ensure that wellness plans are aligned with overall medical plans of careCollects, documents, and maintains member information to ensure compliance with current state and federal accreditation standardsProvides feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective mannerPerforms other duties as assignedComplies with all policies and standardsEducation/Experience: Requires an Associate's degree and 2 – 4 years of related experience.
