Responsibilities:
Lead evidence-based, health education programs such as Chronic Disease Self-Management, Diabetes Self-Management, National Diabetes Prevention Program, tobacco treatment and others to improve community health.Collaborate with the team to create and distribute marketing and communication materials such as brochures, newsletters, email campaigns, social media posts, and health promotion resources to engage the community.Provide administrative support through telephonic services for callers seeking resources via MaineGeneral’s Resource Hub.Deliver community health education in a variety of settings, including schools, healthcare providers, patients, and community groups, to promote healthy lifestyle choices and disease prevention.
