The Health Equity Strategist will lead the development and execution of PacificSource’s community health strategies and related programs and partnerships, specifically related to health equity. The Strategist will focus on critical contractual deliverables, organizational strategic plan initiatives, and provide consultation and expert guidance on health equity and related topics. The Strategist will facilitate discussions, trainings, and projects internally and externally to address the root causes of health inequities. This will involve building capacity to integrate community insights into efforts to develop system level changes that advance health equity in PacificSource’s Coordinated Care Organizations (CCOs), which may include its governance structure and provider networks.

Essential Responsibilities:

Develop and execute PacificSource’s Health Equity Plans in each regional CCO, in close collaboration with the Community Health and regional CCO teams.Convene and facilitate community discussions to gather insights that inform system changes that advance equity, including input for the development, implementation, and monitoring of the CCO’s Health Equity Plans.Support development and implementation of health equity related Transformation and Quality Strategy and Performance Improvement Project activities and reporting with engagement of health councils and community partners.Implement community-level transformation activities that address health equity, the social determinants of health, and national Culturally and Linguistically Appropriate Services (CLAS) Standards to better serve diverse memberships.Support implementation of the Oregon Health Authority’s health equity training requirements.Develop and maintain positive relationships with a wide variety of community members and entities (e.g., organizations representing diverse populations, schools, law enforcement, and social service agencies) that endeavor to improve health equity within the communities served by the CCO.Collaborate and participate on CCO and community led boards and workgroups to bring an equity lens to community health strategies and initiatives that impact PacificSource CCO members.Collaborate with regional CCO teams and the Tribal Liaison to provide support for community events such as health fairs, cultural celebrations, and community festivals.Work closely with the Health-Related Social Needs Program Manager and other members of the Community Health and regional CCO teams to align efforts, internally and externally, and on CCO contract deliverables.Work closely with regional CCO teams to engage community partners representing populations that experience health disparities; to advocate for and ensure meaningful and effective relationships to ensure these groups are not only represented but actively engaged in collaborative decision making with the regional Community Advisory Councils and other CCO governance entities.Consult with internal teams and departments on health equity related projects, including collaborations with the Analytics team to develop and utilize data dashboards to address health inequities and aligning with Dual Special Needs Plan members and the Health Equity Star measure.

Supporting Responsibilities:

Meet department and company performance and attendance expectations.Participate in and support project teams led by other departments and provide necessary input to support the goals of colleagues.Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: Minimum of four (4) years of experience in the health care industry, provider organization, or related area. Relevant experience in the following areas: community health, health equity, diversity, equity, inclusion, and belonging efforts, strategic planning, quality improvement, strategic relationship development, population health improvement. Demonstrated ability to develop and execute strategy required. Experience leading large multi-stakeholder initiatives highly desired. Experience in managing complex projects or project management certification is desirable.

Education, Certificates, Licenses: Bachelor’s degree in healthcare, business, or related field, or equivalent years of additional relevant experience in lieu of degree required. Diversity, equity, inclusion, and belonging experience preferred. Bi-lingual, bi-cultural preferred.

Project management and implementation to reach desired outcomes. Skilled in relationship building and community collaboration. Knowledge in presentation development and writing reports preferred. Organizational skills with solid experience in using computers and various software applications. Excellent public relations, presentation, and interpersonal skills. Demonstrated successful communication skills, including public presentation, training, and meeting facilitation. Maintain high level of knowledge of company products, health reform trends at the Federal and State levels, and the insurance industry. Demonstrated skills with the following software: Microsoft Word, PowerPoint, Excel, and Visio. Strong analytical and problem-solving skills.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 25% of the time.
