Health Information Exchange & Population Health Management Specialist

The Health Information Exchange (HIE) & Population Health Specialist plays a vital role in managing data connections between healthcare systems and external partners. This individual ensures seamless and accurate data flow, resolves connection issues, implements and audits data integrations, and supports population health initiatives. The role also includes collaborating with practices and clinical staff to optimize the use of a population health management platform.

Employment Type:  Full Time, 80 hours every 2 weeks

Requirements:   Bachelor’s degree in Health Informatics, IT, Public Health, or a related field (or equivalent experience) required, 2+ years of experience in health information exchange, population health tools, or healthcare data systems required, Familiarity with HL7, CCD, FHIR, or similar data exchange standards preferred.

Connection Management & Troubleshooting:

Monitor, maintain, and troubleshoot interfaces and data feeds related to health information exchange.Act as the primary liaison with vendors and IT teams to resolve data connection issues promptly.

Implementation & Testing:

Lead or support the implementation of new data connections or enhancements.Test new functionality in the population health management tool to ensure accurate performance and usability.

Data Auditing & Quality Control

Perform audits on outbound data to ensure integrity, completeness, and compliance.Conduct supplemental data audits as requested by internal or external stakeholders.

Practice Engagement & Support

Train and support clinical and administrative staff in the effective use of population health tools.Gather user feedback and collaborate with developers or vendors for improvements.

Documentation & Compliance

Maintain documentation on data flows, testing results, audits, and troubleshooting activities.Ensure adherence to HIPAA and other relevant data security and privacy standards.

Qualifications:

Bachelor’s degree in Health Informatics, IT, Public Health, or a related field (or equivalent experience).At least 2 years of experience in health information exchange, population health tools, or healthcare data systems.Familiarity with HL7, CCD, FHIR, or similar data exchange standards preferred.Understanding of healthcare quality metrics and programs including HEDIS specifications and quality reporting requirements.Strong problem-solving skills and attention to detail.Excellent communication and collaboration skills with technical and non-technical teams.
