The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

Objectives:

Collaborate with facilities to retrieve and organize patient records.

Assist in analyzing trends related to hospital admissions and readmissions.

Job Description

Responsibilities:

Index electronic patient consents and HIPAA authorizations from WellBe’s internet platform.Initiate EpicCare Link access with other healthcare providers and monitor timely removal of access.Work within Fresh Service ticketing to complete assigned tasks.Utilizes HIM databases and spreadsheets to track work performed.Receive and process requests for patient health information according to company and facility policies.Maintain confidentiality and security of privileged information.Create digital images of paperwork for electronic medical records.Respond to requests for patient records, both within the facility and from external sources.Interacts with management, physicians, and staff by obtaining and communicating essential data and assisting other team members to coordinate efficient transactions of patient-related documents.Maintains a regulatory/compliance environment by following organizational policies and procedures to ensure compliance to state, local, and federal standards, and regulations. Strong attention to detail and organizational skills.Knowledge of medical privacy regulations and HIPAA standards.Willingness to learn and contribute to our patient-centered care approach.

 

Educational Requirements

High school graduate or equivalentBasic understanding of records management

 

Skills & Competencies
