Health Information Management Chart Deficiency Analyst

Job Summary

The Chart Deficiency Analyst will review each discharged patient's record for incomplete or missing items, identify the responsible practitioner for each of these items & link the deficiencies to the appropriate physician for completion. The analyst will be responsible for verifying chart completion & monitoring the applicable queues. The position will also be responsible for timely notification to physicians of pending records that need completion, identifying those physicians who will be placed on delinquency listing & notifying appropriate hospital personnel/departments of delinquency issues.

 

Responsibilities

• Reviews each record for missing documents, missing signatures, or missing text according to procedure.

• Assigns appropriate deficiencies to physician(s) according to procedure.

• Maintains analysis turnaround within required time frame.

• Generates physician notification letters according to policy.

• Answers questions the physicians may have about deficiencies & how to access assignments/complete deficiencies in EMR or directs to appropriate person for clarification.

• Monitors "reanalysis queue" daily & reassigns deficiencies when appropriate.

• Verifies physician dictation on the dictation system if physician indicates delinquent records have been dictated & dictated report is not in EMR.

• Provides reports to hospital personnel/departments on incomplete records & physician delinquencies according to procedure.

• Maintains incomplete record rate below 50% of discharges per JCAHO standards.

• Uses initiative & creativity to encourage physician compliance with chart completion policies.

• Performs chart reviews as needed.

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Competency Statement

Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist.

Education 

• High School Diploma or GED (Required) Experience: 2 years Substitution: Successful completion of related program pertaining to the medical records/health information area. May substitute for 2 years experience.
