Health Information Management Specialist II

POSITION SUMMARY:

The Health Information Management Specialist II reports directly to the Records Manager and supports all basic functions of the Health Information Management Section. This position is accountable for reviewing, analyzing and assembling medical record documentation (automated and hard copy) and ensuring accuracy of record types and demographic data including following-up on deficiencies in documentation according to departmental policies and procedures. Monitor daily provider document deficiencies and communicates to providers both in written and electronic form.  Validate daily Master Patient Index data utilizing custom software.

 

EDUCATION/CERTIFICATION:

This position requires a minimum of a High School diploma or GED. 

EXPERIENCE:

Two years of hospital and/or medical office experience with related field of work preferred.

COMPETENCIES:

Basic computer skills are needed for daily use of Microsoft windows based programs such as Outlook, Word, Excel and internal Intranet and Meditech programs. Must be able to read, speak and write proficiently in English in order to effectively communicate on a daily basis with hospital staff, patients and visitors.  Occasional travel between ECHN business sites is necessary, making reliable transportation a requirement.

Notes any and all provider deficiencies electronic and paper. Completes electronic deficiency notice with 95-100% accuracy. Reviews in-patient and ambulatory records. Completes deficiency analysis timely and within daily production standards. Ensures the appropriate reports and signatures have been designated to be completed by provider with 95-100% accuracy. Refers cases in question to supervisor. Files completed charts in terminal digit order in permanent file area after charts have been completed through deficiency analysis. Incorporates incomplete charts into unit file folder. Reviews all records subsequent to provider completion to verify appropriate reports and signatures, previously noted have been completed at a level of 95-100% accuracy.  Refers cases in question to supervisor. Monitors daily OR schedule. Notes deficiencies for non-compliant Operative dictation and notifies Physician office. Documents misdirected faxes. Monitors daily electronic signature reports. Updates electronic deficiency assignment. Assists providers in the process of completing the medical record as observed by the supervisor or feedback. Request assistance from supervisor when needed. Completes weekly incomplete record notices, warnings and suspension to providers.  Maintains notice file.  Reports weekly notices to HIM Records Manager. Reports weekly incomplete charts to HIM Records Manager. Completes daily MR# and EPI# merges.  Record retrieval and follow up to paper and electronic chart. Maintains MPI file of merges. Monitors manual transcription distribution.  Provides assistance to manual distribution. Completes entry of daily productivity log. Maintains file for quarterly review.
