Position: Health Information Technical

Direct Report: HIM Team Lead

Position Summary:

The purpose of the Health Information Technician is to represent the Medical Records Department by performing exceptional quality customer service and overall support of the medical records file coordination in compliance with federal, state, local requirements and accreditation, as well as according to hospital policy.

Primary Responsibilities: Assist both internal and external customers with exceptional customer service skills. Coordinate loose documentation: Verify loose documentation is part of the medical record. Return documentation that does not belong in the medical record to the appropriate author, department or disposal method according to hospital policy. File in proper patient’s chart. File in proper location of the chart. Procurement of records: Process records of expired patients. Purge records to offsite storage according to purge schedule. Prepare records for destruction according to destruction schedule. Copy transfer charts. Process fax requests for information. Perform data entry for accurate record filing and retrieval to offsite storage system. Provide notary services. Coordinate record out-guide system: Locate. File. Retrieve. Out-guide. Produce labels for permanent folders. Process transcription services STAT documentation for timely completion. Prepare chart list and retrieve charts for internal and external customers. Process the following Release of Information (ROI) requests in a timely manner according to HIPAA regulations. Determine records to be released by reviewing requestor information. STAT requests. Denial requests. Audit requests. Escalate all internal/external customer issues to appropriate leadership that may delay the timely release of records. Perform other duties as assigned.  Desired Job Qualifications/Skill Sets:  High School diploma or equivalent. Work independently and in a team environment. Strong auditory, verbal and written communication skills. Organize, prioritize and multi-task responsibilities. Strong computer skills. Certified Notary Working Hours/Conditions: Saturday coverage is determined by a rotation schedule. Medical Records Department will be closed on Sunday’s and Aultman observed holidays. Lunch periods and all breaks must be coordinated with other Medical Records Technicians in section to maintain adequate staffing at all times. Moderate physical activity requirements. Subject to frequent interruptions. At times, noisy and crowded. Physical Activity Requirements: Work Positions Stand – approximately 80%. Walk – approximately 10%. Sit – approximately 10%.   Qualifications Climb ladders/stools – moderate. Climb stairs – minimal. Kneel – significant. Reach overhead – significant. Push/pull loads – significant. Twist – moderate. Bend – significant. Lift frequency – significant. Lift – up to 50 lbs.
