Position Summary
 
Job Description

The Health Plan Quality Community Health Worker is responsible for collaboration with members and provider groups to promote quality gap closure through education and reporting. The Health Plan Quality Community Health Worker will use their experience in, and knowledge of, the communities they serve to identify and develop plans to overcome barriers to quality care.

Key Outcomes:

Creates a positive experience and relationship with members and providers.Empowers patients by assessing their needs and connecting them to appropriate resources and services.Independently develops and delivers health education tailored to the community’s needs, considering literacy levels and cultural preferences.Identifies gaps in knowledge and provide accurate, actionable health information without direct oversight.Provides culturally appropriate health education and facilitate access to care while respecting patient autonomy.Identifies and address urgent needs (e.g., housing food insecurity) by leveraging community resources and exercising independent judgement.Manages crises with minimal supervision, ensuring appropriate referrals and support.Provides annual updates on quality measure technical specifications to provider groups.Delivers quality gap opportunity reports to provider groups.Coordinates collection, analysis, and reporting of quality project deliverables. Participates in on-going education and training to improve skills and role-specific certifications or specialization.Maintains growth and development through self-directed learning activities and involvement in ongoing community health worker trainings.Participates in group meetings to ensure policies, procedures and workflows are up to date and advises on revisions needed.Performs other duties as assigned.

Education/Experience:

Associate’s degree in allied health discipline2+ years of experience working in human services or community health related fields within the local communityExperience with care coordination and/or care managementExperience related to SDOH and Health Equity

Required License(s) and/or Certification(s):

Community Health Worker Certification preferred.

Skills/Knowledge/Competencies (Behaviors):

Demonstrates understanding of and alignment with Martin’s Point values.Knowledge of culture and values of community and familiarity with the resources available in the communityExcellent written and verbal communication skills.Working knowledge of Microsoft Office applications.Excellent organizational skills.Experience with motivational interviewing.Ability to utilize an electronic medical record.Proficiency of medical terminology.Understanding of the managed care environment.Demonstrated understanding of community services and navigating the healthcare system.
