Health Services Coordintor (LPN)
Job Description The Health Services Coordinator is responsible for supervising and implementing the overall health care plans for all clients in the Residential programs. This position is also responsible for the observation and training of all staff who dispense medication to clients in the home. The Health Services Coordinator is also responsible for the assessment, diagnosis, outcome identification, planning, implementation and evaluation of the medical and mental health (primarily psychotropic) care of the clients, which includes setting up and taking clients to all appointments. **Major Job Responsibilities** **:** 1. Implement, coordinate, monitor and evaluate the health care of each client. 2. Coordinate, communicate and consult with physicians and other health care professionals in the community. 3. Function in accordance with the Minnesota Nursing License code of ethics and has supervisory responsibility to oversee the administration of medications and the proper use of medications for the client. 4. Communicate responsibilities regarding medical issues for the client’s family, staff members, social workers or other appropriate persons. 5. Maintain departmental relationships in a way that will assure safe and effective health maintenance and health care for the clients. 6. Provide assessment and monitoring of pharmaceuticals used by the clients to include over-the counter medications, psychotropic medications and prescription medications. 7. Schedule, coordinate and document psychotropic med sessions to include quarterly psychotropic medication reviews and monitoring side effects. 8. Perform the role of liaison nurse between the supervisors and health care professionals. 9.
