Health Unit Coordinator - Birchwood Senior Living

Birchwood Health Care Center, a Lifespark Skilled Nursing Community, is looking for dedicated and passionate Health Unit Coordinator to join our growing team.

Birchwood Health Care Center is home to seniors requiring TCU (Transitional Care Unit) Care and Long-Term Care. We are in Forest Lake, near North Shore Dr and Forest Lake. We have been helping seniors Age Magnificently since 1979 and are part of Senior Care Communities, a local non-profit supporting seniors.

Schedule: 10:00am-3:00pm, M-F
Pay: $21-26

In this position you will provide clerical and clinical support, communication, and assembly and maintenance of all resident medical record charts.  Duties include:

Assemble the appropriate paperwork for resident records, prepare admission labels, and provide additional clarification of orders. Review and process completed paperwork, order resident medications, and follow up on documentation and future appointments. Communicate with resident’s family (i.e. upcoming clinical appointments and transportation arrangements). Request information as needed or required such as hospital discharge summary, history and physicals, and physician clinic progress notes. Prepare paperwork and referrals for transfers, and outpatient visits. Maintain daily filing and keep charts thinned, removing any unnecessary information. Update face sheet information as needed. Routinely audit records (i.e. Admissions, Twenty-three-day, Three-month, Hospital Returns, etc.) for regulatory compliance according to policy. Monitor information sent to the physician (i.e. physician orders, history and physical reports, progress notes, etc.). Make appointments for outpatient visits, arrange transportation, and notify appropriate family members. Monitor physician visits and notify MD of visits due via phone/fax or notify Director of Health Information of overdue visits according to policy.

 Preferred Qualifications:

Health Unit Coordinator experience, preferably with long-term care experience. Working knowledge of medical terminology. Exceptional telephone and communication skills and the desire to work closely with senior adults is essential. Must possess proficient computer skills, including email use, Microsoft Word, Excel, and other related software programs related to job duties.
