With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system. *_Position Summary:_* Hartford HealthCare’s Heart & Vascular Institute is rapidly growing, with the anticipation of further expansions and programmatic development across the state and beyond. Reporting to the Director of Specialty Programs, Heart & Vascular Institute, the Program Manager is responsible for the design, implementation, development and integration of clinical programs, as well as Institute-wide research and education programs, to ensure a multidisciplinary care approach and successful programmatic growth and expansion across HHC. Serves in a key administrative lead role, partnering with the Institute’s physician leaders and administrative staff to establish one programmatic standard of care across the Heart & Vascular Institute. Ensures alignment with best practices for optimal standardization, integration, growth and quality across the system. Successfully lead teams of physicians, administrative leaders and staff across multiple sites and work in a collaborative style within a matrixed, complex medical environment to achieve Heart & Vascular Institute goals. The Heart & Vascular Institute seeks to meet and exceed national benchmarks for quality, value and outcomes, and create new, innovative models of care delivery while fostering and maintaining relationships with the communities we serve. *_Position Responsibilities:_* *Key Areas of Responsibility* 1. Partner with physician and administrative leaders to develop, implement, and expand multidisciplinary programs across the state and beyond. 2. Provides program management for Heart & Vascular Institute program development and support for workgroups, committees and annual initiatives across HHC. 3. Partner with physician and administrative leaders to expand the Heart & Vascular Institute Research Program for enhanced clinical research trial management. 4. Partner with physician and administrative leaders to support educational offerings including regularly scheduled series (case conferences, M&Ms, etc.), programs, conferences and symposia. 5. Lead integration initiatives for programmatic expansion and incorporation of newly hired and/or acquired practices and providers. 6. Coordinate, participate, mentor and leads HHC organizations and stakeholders to engage in annual goals, milestones and requirements as it relates to HHC’s Heart & Vascular programs. Partnering with the Quality team as appropriate. 7. Engage physicians and staff across the system to align with clinical treatment standards and quality and outcome measures to ensure a single standard of care across HHC. 8. Collaborate and partner with other providers across HHC and in the community, as well as local/state/national organizations, to provide comprehensive care options to patients and their families. 9. Engage the community and ensure Heart & Vascular Institute presence at local and regional disease education and awareness events related to the program(s). 10. Understand key market opportunities and trends to support market expansion and programmatic growth strategy. 11. Recommend and develop internal policies and procedures related to specific program needs. 12. Participate in strategic, short range and long-range planning for the Heart & Vascular Institute programs. 13. Support all aspects of system-wide HVI program growth including provider development, marketing, quality, and safety initiatives. 14. Collaborate, develop, and share work plans and processes as it relates to HVI program management. 15. Develop and sustain internal and external relationships to support programs within the Heart & Vascular Institute. 16.
