Job Summary
The Heart Failure Program Coordinator is responsible for the coordination and maintenance of a comprehensive Heart Failure program that complies with national standards for the deliverance of excellent quality of care for Heart Failure patients as dictated by the American College of Cardiology (ACC), and governmental regulation for Medicare Compliance. The Coordinator partners with the physician leaders of cardiology services and other related service line and clinical program leaders to ensure development of a comprehensive, market competitive, and leading edge clinical program at Baptist Health Louisville and at any other affiliated sites across the region as deemed appropriate if/when applicable.

Minimum Education, Experience, Training and Licensures Required*
Bachelor's Degree required
RN License required
5 years experience required
BLS and ACLS required. Cardiology specific certification preferred.
