Heart Failure Program Manager

Position Summary

The Program Manager, Heart Failure plans, coordinates, evaluates, and promotes the system’s heart failure program under the guidance of the Clinical Analytics Manager. The Program Manager, Heart Failure works closely with the Cardiovascular Medical Director(s) and Nursing/Other Clinical Directors and Managers. This position collaborates with nursing, medical staff, and ancillary areas to ensure evidence-based care is provided to patients throughout the continuum of care and acts as the heart failure subject matter expert responsible for summarizing findings of clinical quality initiatives for outcomes reporting in addition to synthesizing current evidence-based research to apply to the heart failure certification and accreditation programs. In addition, the Program Manager, Heart Failure acts as an internal quality consultant and subject matter expert in clinical effectiveness, process improvement, physician peer review, certification and accreditation readiness, and program management.  This position is responsible for uncovering opportunities for improvement at a system and individual campus level through data mining/analysis and observation of processes of care in heart failure.  This position is responsible for ensuring all heart failure project activities are in alignment with organization’s strategic goals while personnel, finance and business consequences are not only considered but are maintained and optimized. This position supports the healthcare system by maintaining an environment of continuous survey readiness for all heart failure-related certification and accreditation programs.  Additional responsibilities as assigned by the Clinical Analytics Manager.

 

Minimum Requirements

 

Education           

Master of Science in Nursing

 

Experience        

5-7 years Cardiovascular or Similar Nursing Experience

 

License/Registration/Certifications       

SC Nursing License/Compact State Board Certification in Area of Clinical Expertise

Core Job Responsibilities

Ensures system compliance with professional, regulatory, and governmental standards of care. Responsible for planning, conduction and coordinating leadership and staff meetings related to the system heart failure program. Demonstrates knowledge of equipment or other material needs, which includes purchase, maintenance, repair, inventory, and replacement. Develops and supports critical annual operating goals that align with the strategic focus of the organization. Demonstrates knowledge of budget preparation and use.  Demonstrates commitment to professional growth by attending or participation in management training and /or continuous education on a regular basis. Develops, implements, measures, and monitors success of plans designed to improve associate professional educational levels on core competencies. Demonstrates ways to enhance the quality of communications between the department and its customers, fellow associates, other departments, physicians, and the community. Fosters and promotes teamwork within the department and the organization. Resolves conflicts constructively, and in a timely manner. Demonstrates appropriate computer-based skills commensurate with job duties.

 

Heart Failure Accreditation Responsibilities

Lead the development of system protocols and guidelines for patient management based on current evidence-based standards of care. Develop, implement, and evaluate comprehensive, system-level patient education programs. Participate in the development and execution of quality assurance programs and projects at both the system and individual campus level. Assure that the Heart Failure Management program remains in compliance with DNV, ACC/AHA, and other government agency guidelines. Participate in the development and implementation of research protocols and interpret the protocols for participation of patient and family members. Develop, review, revise annual staff education. Participate in teaching activities at for physicians, nurses, allied health professionals and for students in those disciplines. Participate in teaching activities at for physicians, nurses, allied health professionals and for students in those disciplines as requested Develop and present information to marketing, community, and professional groups regarding heart failure management. Offer screenings and education in the community. Coordinate and offer comprehensive heart failure specific education for community and non-profit organizations in the service area. Provide culturally appropriate education and materials to the community. Assist in the development of education to help ensure that patient and family demonstrate knowledge of health status, treatments, symptom management, skills, medications, and adaptive behaviors gained as a result of teaching interventions. Chair multidisciplinary team to govern heart failure program.  Actively collaborate with the designated Heart Failure Coordinators at all campuses to ensure a system approach to heart failure care, while recognizing the individual and distinct needs of each campus. Act as the EMS liaison for heart failure. Develop communication policies and protocols for pre-hospital personnel. Provide educational offering to EMS providers. Act as the Heart Failure Clinic(s) / MGC office liaison to assist with transitional policies and protocols. Ensure that risk stratification protocols are in place and are being utilized for decision making related to the heart failure patient. Serve as an expert resource in heart failure processes to healthcare system. Participate as a core member of the Heart Failure Readmission Taskforce committee. Must be able to withstand the stress and demands of an active position. Must be able to withstand many long hours of standing, walking, stooping, bending, and sitting. Must be able to read, write, and speak fluent English. Must be able to respond calmly and efficiently to crisis situations. Must be able to work as a team member with peers, other department managers, staff, and administration. Manual dexterity, good eye-hand coordination and adequate vision needed in daily work.
