Day - 08 Hour (United States of America) **This is a Stanford Health Care job.** **A Brief Overview** Under general supervision, to perform health information management clerical activities such as reviewing medical charts for deficiencies and assisting medical staff regarding completion of charts. Application of specialized knowledge of medical chart format, health information management procedures, hospital organization and medical terminology. Incumbents are expected to have sufficient judgment to solve complex clerical office and chart locator problems. **Locations** Stanford Health Care **What you will do** + Obtains, reviews, and analyzes outpatient medical record discharges for completeness: + All medical records are accurately retrieved and assembled within discharge. + Identifies deficiencies by physician for each out patient discharge medical record. + Performs data entry of chart deficiencies as indicated by the software are program. + Responsible for keeping within productivity standards set by the supervising staff for analysis and assembly of outpatient discharges. + Records are accurately filed in the incomplete file immediately up on completion (initial/recheck). + Monitors status of incomplete chart work: + On a weekly basis remind physicians of incomplete work per departmental suspension process. + Prepares statistical reports, maintains logs, files and computer data files on the completion of incomplete charts by physicians. + Reports variances or physician concerns to the Supervisor or Director. + Prepares completed charts for permanent medical records files: + Pulls and verifies charts for physician completion within the HIM department. + On a daily basis send charts to the GI lab and OR recovery room for physician completion according to a pee-determined schedule. + Updates tracking software removing completed charts deficiencies, + Tracks the location of incomplete charts consistently in the chart locat system. + Maintains, researches and corrects all information for incorrect or double numbers in a timely manner. + Performs extended chart or information searches and successfully locates most files in a timely manner. + Releases patient information appropriately: + Releases incomplete records per departmental guidelines. + Interviews parents to obtain information for preparation of official birth and death certificates: + Completes birth and fetal death certificates in a timely and accurate and departmental guidelines. + Obtains required signatures as appropriate and files completed documents with the county within required state deadline of IO days. + Reads and understands medical terminology to the degree necessary to format and consistency in the general contents of a medical record. + Cross trained to provide coverage in the Livermore office for: + Pulling charts according to the surgery schedule. + Assembly and analysis of skilled nursing, GI, and psychiatric discharges. + Chart management procedures specific to the Livermore campus. + Monitors dictating system and transcription system for completed reports: + Responsible for the distribution, filing and tracking of all dictated reports. + Update incomplete charts as needed when new dictated reports are filed. + Responsible for taking part in the copying of records for billing purposes as well as patient requests. + Will participate in the filing of a pre-determined portion of loose paperwork maintained within the HIM department. + Works effectively and harmoniously with others. + Works independently and exercises initiative, judgment, and tact. + Performs other job duties as assigned by supervising staff. **Education Qualifications** + High School Diploma or GED **Experience Qualifications** + Two years experience in a clinic or hospital setting **Required Knowledge, Skills and Abilities** + Basic computer skills, including basic knowledge of how to access and retrieve computerized data. + Knowledge of JCAHO/Title 22 compliance standards as pertains to chart completion preferred. + Basic knowledge of State and Federal laws pertaining to patient confidential information and the requirements for release of confidential information preferred + Excellent public relations skills. + Knowledge of general office practices and procedures including, but not limited to, knowledge of modern standard office machines including computers, fax, copier, and typewriter. **These principles apply to ALL employees:** **SHC Commitment to Providing an Exceptional Patient & Family Experience** _Stanford Health Care sets a high standard for delivering value and an exceptional experience for our patients and families.
