HIS Tech II

The Health Information Services (HIS) Technician manages and ensures the integrity of patient chart information. All medical records are maintained in an appropriate manner in accordance with current federal, state and local standards, guidelines and regulations that govern medical records. An established procedure will be used to assure necessary and accurate information is on all charts. The Technician enters and retrieves all patient chart data, corrects missing or incorrect data and oversees the dissemination of patient chart information to physicians, patients, and other requestors. This role also performs clerical and customer service responsibilities including using office technology, answering and routing phone calls and processing paperwork.

 

PRIMARY DUTIES AND RESPONSIBILITIES

 

Follows up with nursing staff and revisits patient care units and when 100% of discharged records are not obtained. Reviews records for correct admission and discharge date, patient type, medical record number and FIN numbers; notifies HIS Manager when incorrect or duplicate information is identified. Demonstrates the ability to accurately create bins/bin labels and loose batches utilizing ScanTrak. Analyzes all patient types in assigned analysis Task Queues within 5 days post discharge 95% of the time. Analyze all patient types with a 98% accuracy rate. Accurately completes Vital Record requirements on all Newborns and/or Paternities as needed and submits electronically via GAVERS. Register birth, fetal and infant death certificates into GAVERS accurately and timely according to state guidelines. Handles inquiries regarding Birth Certificates and related issues within 48 hours of notification by email, phone call; message or presenting individual. Manage time effectively to enter name changes in STAR no greater than 5 days post discharge. Employee demonstrates behavior that supports our patient’s experience through direct patient care or supporting the departments that provide direct patient care. All employees will contribute to NSH’s scoring above the national benchmark for the publicly reported HCAHPS data. Preps all patient records according to established departmental policies for patient types. Preps all loose work documents by looking in Cerner or STAR to record the patient’s. Account/FIN number for proper processing. Scan discharges and loose filing of each patient type received daily.

REQUIRED:

A high school diploma; one year of medical records/healthcare experience; One year of business office knowledge (i.e. telephone and fax) and proficient typing skills. Working knowledge of operating a PC to include use of Microsoft Word, Excel, and Access. 4. Good verbal and written communication skills.
