With five state-of-the-art medical facilities, we’re committed to holistic and equitable care—and we're looking for someone like you to help us make a lasting impact. As a key member of our outreach and education team, you will support individuals in accessing PrEP/PEP and STI screening, while promoting HIV awareness and prevention across diverse communities. Your ability to build relationships, deliver culturally competent education, and coordinate essential care will help ensure patients are informed, supported, and connected to the services they need most. What You’ll Do: + Provide patient education and support around PrEP, PEP, HIV testing, and STI screening + Conduct assessments and develop care plans to meet individual prevention needs + Coordinate outreach efforts at community health fairs, shelters, events, and more + Facilitate referrals and link clients to supportive services and resources + Maintain accurate and confidential records within the EHR system + Case conference with medical providers and participate in interdisciplinary team planning + Deliver services in-person and via telehealth as needed + Advocate for high-risk and underserved populations to reduce health disparities ?Qualifications: + Bachelor’s Degree in Social Work, Psychology, Human Services, or related field + Experience in HIV counseling/testing, PrEP/PEP services, and community outreach + Strong understanding of care coordination and harm reduction principles + Excellent interpersonal and communication skills + Proficiency with Microsoft Office and EHR systems + Language skills in Spanish, French, or Haitian Creole are aplus What We Offer: + Competitive salary and generous health benefits + Work-life balance with supportive team culture + Opportunities for professional growth and training + A chance to be part of a mission-driven organization making real change If you’re driven to serve, educate, and empower—especially in communities most impacted by health disparities—we want to hear from you.Apply today and help shape the future of community health in Brooklyn.
