POSITION SUMMARY

The HIV/PrEP Navigator is a grant funded position. The purpose of this position is to provide direct case management of all assigned patients; thereby contributing to the improvement of patient care, their quality of life, and treatment compliance. The HIV/PrEP Navigator effectively utilizes case management to coordinate with clinical team members to ensure that effective patient care services are provided to patients with complex medical and/or psychosocial needs. In this role the incumbent organizes the patient’s entry into and through the patient care system; coordinating patient care services and serves as a patient advocate ensuring delivery of whole health care to patients.
