Overview
The RAP Peer provides peer support, information, and advocacy to individuals at diagnosed with HIV. Candidate will possess a strong understanding of HIV. Must be a current or past consumer of mental health, substance use, co-occurring behavioral health services, and HIV.
• Outreach: Conduct regular outreach targeted at reaching people at risk for or living with HIV. Outreach activities will include HIV and HIV prevention education and community testing events.
• Patient Case Management: Work closely with patients diagnosed with HIV to access care and treatment. Assess participants for social determinant of health such as lack of insurance, transportation issues, housing, food insecurity, etc. Work with Team to resolve these issues such as linkage to care services, and providing applicable referrals/referral follow up. Assist RAP Specialist with medical appointment and lab appointment reminders.
• Data collection: Maintain accurate documentation of program activities. Document all patient progress in electronic medical record.
• Education: Attend all required trainings and any additional training as needed to improve skills and knowledge surrounding HIV.
Clinical/Department Competencies- Knowledge of HIV disease progression and current treatments, HIV retention and adherence.
