clientsrsquo families Provides personal care and housekeeping assistance to hospice clients in their homes follows the established plan of care under the direction of the client care coordinatorIn general most of the time will be spent in the following activities Provides personal hygiene and grooming care to hospice clients Provides meals for hospice clients nutritional needs Reports changes in hospice clients condition to client care coordinator or supervisor Performs routine housekeeping and home management duties Records each hospice clients care on care plan and submits care plan in a timely manner Assists in providing the unique emotional support needed for the hospice client and family Responds sensitively to the multiple complex needs of a terminal client which includes providing end of life care Assists with routine therapy exercises Reminds hospice client of medication regime opens medicine containers for clients as needed Participates in case conferences as
