Records and maintains timely and accurate referral data. Provides clinicians with required information needed to start services for a new patient, and contacts physician for any concerns. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Previous home care experience preferred. 2. Graduate from an accredited school of nursing. 3. Current license as a registered nurse. 4. Familiar with state and regulatory requirements. 5. Strong verbal and written communication skills. Strong interpersonal skills. Strong problem solving skills. Strong time management skills.
