Responsibilities:The Home Care Liaison functions as the primary point of contact for referral sources when referring patients to Community Home Care. Responsible for developing and maintaining positive relationships with referral sources. Reviews patients' medical records, interviews patients/families, and consults with staff that are referring patients to Community Home Care. Acts as a liaison between the patient/family, physicians, and Community Home Care to assure appropriateness of patient for admission. Accountabilities 1. Develops and maintains professional relationships with multiple referral sources.2. Performs liaison activities between Community Home Care and referral sources.3. Autonomously acts to educate and mentor referral sources on the appropriateness of admission to Community Home Care.4. Reviews home care referrals for appropriateness using established criteria for admission.5. Conducts pre-admission interviews and independently evaluates the appropriateness of referrals.6. Educates potential patients on home care, including their rights and responsibilities. Obtains agreement or refusal for home care. Confirms patient demographics.7. Works with referral source and attending physician to obtain orders for home care.8. Communicates effectively and timely with Community Home Care Intake Coordinators and Schedulers to assure proper scheduling of the patient for a home care visit.9. Actively promotes Community Home Care and its programs and services to potential referral sources, patients, families, and to the community.10. Performs other job-related duties as assigned.Qualifications:Education• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required• One of the following is requiredo Graduate of an accredited Vocational Nursing, Physical Therapy Assistant, or Occupational Therapy Assistant Program OR qualifies for state licensure through the process of challenging the licensing examo Associate's Degree in a relevant field Experience• 2 years of relevant healthcare experience required• Experience in Home Health preferred Licenses and Certifications• DL - Valid California Driver's License and driving record acceptable to Community Health System required• One of the following is preferredo LVN - Current State of California Licensed Vocational Nurse Licenseo PTA - State of California Physical Therapist Assistant Licenseo OTA - Current State of California Occupational Therapy Assistant LicenseDisclaimer:
