Salary: $88K+/year + $2,500 Sign-On Bonus
Service Area: North Central (coverage area: Golden Valley, Robbinsdale, Brooklyn Center, Brooklyn Park, Anoka, Coon Rapids, Oak Grove & surrounding). Plus mileage reimbursement of .67 cents per mile.
Schedule: Monday – Friday, 8:00am-5:00pm (with a holiday rotation and NO WEEKENDS!)

Primarily responsible to plan, organize, direct home care services. Experience in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Qualifications:
2 years’ RN experience in health care (preferably home care or hospice) Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred Experience in customer service/direct member contact field preferred Experience supervising caregivers Knowledge of Medicare Home Health & Hospice regulations Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice. Graduate of an accredited school of nursing Current Minnesota Nursing License Must pass a MVR check based on Lifespark company policy
