✨ About the Role:
Trinity Health At Home is seeking dedicated and compassionate Hospice/Home Care Aides to provide essential care and support to patients in New Castle County. In this role, you’ll work under the supervision of an RN, assisting clients with daily living activities while delivering care that promotes dignity and comfort.
💖 Key Responsibilities:
Follow individualized care plans designed by the RN.
Assist with personal care (bathing, dressing, grooming, toileting, and mobility).
Provide light housekeeping, meal prep, and laundry services.
Record vital signs and report any changes in condition.
Support clients with safe use of mobility aids and assist in self-administered medications.
Maintain accurate and timely documentation of care provided.
🎓 Qualifications:
High School Diploma or GED.
Completion of a federally compliant Home Health Aide training program and competency evaluation.
Minimum of 1 year experience as an aide in a home care, hospice, or institutional setting.
Valid Driver’s License with reliable transportation.
Ability to work independently while fostering professional client relationships.
💪 Physical & Work Requirements:
Ability to frequently lift, stand, and walk.
Occasionally bend, stoop, and assist in lifting patients (up to 35 lbs).
Adaptability to various work environments and client needs.
Travel within New Castle County for home visits.
