Penn Medicine Princeton Health is seeking dedicated Home Health Aides to join our team. As a Home Health Aide, you will play a crucial role in implementing personalized care plans and assisting patients with daily living activities in the comfort of their homes. **Key Responsibilities:** + **Patient Care:** Assist with bathing, oral hygiene, skin and hair care, meal preparation, and feeding. Help patients with dressing, using bedpans, commodes, and urinals. Manage Foley catheter drainage and document weekly reports. + **Exercise and Transfers:** Support patients with home exercise programs, ambulation, and bed-to-chair transfers. Ensure patients' comfort by repositioning them in bed. + **Patient Environment:** Perform essential household tasks and environmental services as specified in the care plan. Assist with pre-poured medications and provide respite care for primary caregivers. + **Documentation:** Accurately document patient care in a timely manner according to Medicare/JCAHO standards. Communicate schedule changes and verify care provided in the EMR. + **Communication:** Report any unusual findings, changes in patient status, home safety hazards, or patient/family concerns to the primary nurse or Home Health Aide Supervisor. Facilitate communication between the family, RN Case Manager, and Scheduling Coordinator. + **Plan of Care:** Follow the written care plan, adhering to assigned days and hours for patient visits. Review the biweekly IDT Plan of Care before providing care. + **Employee Travel:** Provide your own transportation to and from patients' homes, meeting state requirements for operating a motor vehicle. + **Education:** Attend annual in-services and complete a minimum of 12 hours of continuing education annually. Adhere to PMAH procedures and meet all competencies defined for Home Health Aides. **Qualifications:** + Certified Home Health Aide (Required) + Valid Driver's License (Required) + Proof of Automobile Insurance (Required) + H.S.
