Responsibilities Personal Care as needed A successful Home Health-care Aide provides personal care services under the direction and supervision of the Registered Nurse Ability to follow a care plan Meal preparation/feeding Laundry Light Housekeeping Knows and follows infection control policies including proper hand washing techniques. Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse. Documents care provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested. Treats patients and families with respect. Maintains confidentiality in all aspects of the job. Accepts and fulfills assignments with the agency. Exercise good judgment in accepting assignments. Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor. Performs other related duties and responsibilities as deemed necessary Qualifications Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided. Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant CNA certification in applicable state OR 1 year of supervised, verifiable experience within the last 3 years. Be at lease eighteen (18) years of age. Demonstrated ability to read, write, and follow a written Plan of Care. Good interpersonal skills and a passion for caring for those in need. Current driver’s license, good driving record, and/or reliable transportation is required. 1 + years of experience is highly preferred. Frequent standing walking, stopping/bending infrequent squatting/kneeling and balancing Weight handled: push, force required: 30 - 50 lbs (at times)
